How We Assist Communities

We work with nonprofits, state and local governments, hospitals, managed care organizations (MCO) Federally Qualified health centers (FQHC), community-based organizations and small businesses that need help launching their initiatives.

We support, identify and build community and systems changes. The Foundation provides ongoing and as-needed technical assistance for a wide variety of public health issue areas, community/population health, policy implementation, agriculture-related projects, housing initiatives, and more. Our team provides customized support and program management, with a focus on the unique needs of rural, minority, hard-to-reach populations, low-income communities, and other marginalized groups.
