Do you know someone whose work deserves to be celebrated on a larger stage? We invite nominations for individuals, teams, and organizations that exemplify leadership, vision, and measurable community impact.
Award Categories Include:
• Farmer of the Year: Recognizing an individual farmer or farming operation demonstrating sustainable agricultural practices, community engagement, and a commitment to local food systems.
• Health Equity Champion: Honoring an individual or organization working to address health disparities and promote equitable access to healthcare and resources within underserved communities.
• Community Health Worker of the Year: Celebrating a Community Health Worker who demonstrates exceptional dedication, compassion, and effectiveness in connecting community members with vital health and social services.
• Community Leader: Recognizing an individual who has made significant contributions to their community through leadership, advocacy, and service.
• Environmental Excellence: Honoring an individual or organization demonstrating a commitment to environmental sustainability, conservation, and protection.
• Faith Community Leadership: Celebrating a faith leader or faith-based organization making a positive impact through community outreach, social justice initiatives, and spiritual guidance.
• Healthcare Innovation: Recognizing an individual or organization developing and implementing innovative solutions to improve healthcare delivery, access, and outcomes within the community.

• A detailed description of their impact and accomplishments
• Supporting stories, data, or examples
